How do I claim for out-patient treatment?
If you’ve paid a bill for out-patient treatment and want to claim it back, please complete our online claim form for out-patient treatment.
If the amount you’re claiming exceeds US$500 (or equivalent in another currency), please ask your doctor to complete Section B of the PDF claim form. You can either upload this PDF as an attachment on the online claim form, or email it to us later at firstname.lastname@example.org. Please provide all the information we ask for, and attach any supporting documents (e.g., invoices, receipts).
- In some cases, we might need your doctor to complete Section B of the PDF claim form even if your claim is less than US$500.
- We can only reimburse your claim once you’ve sent us copies of the fully itemised invoices and receipts. These give us a complete breakdown of all treatment you’ve received and any medication you’ve been prescribed.
- We may request original documentation relating to your out-patient treatment, so please retain all original invoices and receipts for 12 months following your treatment.
