3 easy steps to making an out-patient claim:

Step 1

Choose and download a claims form from the list below. Please ensure all claim forms are completed as fully as possible:

General claim form
Dental claim form
Well-being claim form
Compassionate Home Travel claim form

Step 2

Complete the form as fully as possible ensuring that you give details of your symptoms or what you are claiming for:

  • include your policy number
  • include your contact details, including a valid e-mail address
  • include your Doctor or medical professional’s contact details
  • have Section B fully completed by your doctor/medical professional
  • have listed and provided an explanation of receipts 
  • include original receipts with the form
  • have fully signed and completed the consent form
  • clarify your bank details to ensure swift reimbursement 

Step 3

If your claim is for less than $500

If the total value of the claim amount you are claiming is less than the equivalent of US$500, we will accept scanned copies of the claim, including the receipts. Please ensure you keep the original receipts for a period of two years.

Fully completed forms can be scanned and e-mailed to claims@william-russell.com. Alternatively, you can submit your claim form by post to the relevant address below.

 

If your claim is for more than $500 

If the total value of the claim amount you are claiming exceeds US$500, please check the form is fully completed and submit, along with receipts, to:
